Good thing is,, you like it !! My guess is that it was made mid century or thereabout !! Looks to have modern wiring same as extension cords are today !! Is the plug a three prong or two prong ?? Most today are three prong but mid century would only be two !!

Well,, you will find many lamps on ebay and other sites that are advertised as Leviton !!
Leviton only make the electrical components,switches ect !! The link above is the Leviton website so you can see what I mean !! Other than a few companies most all lamp makers order parts in bulk from different mfrs !! Glass parts from one, ceramic from another, shades from another, wood from another ect !! Then they are assembled at one place and tagged with that companies name !! Not one lamp mfr that I can think of has facilities to blow or mold glass plus a foundry to make the metal parts,,ect !! Even Tiffany had many of their lamp bases made by others !!
That other switch may be a dimmer switch !!

What shade came with it ?? How tall is it ?? Lamps are rarely if ever marked because it takes three companies to make them !! One for the wood or metal uprights, one for the electrical components, and one for the shade !! Only rarely are even two out of three done by one company !!
